Online Admissions Application
PLEASE NOTE: Students wishing to enter the Pharm.D. program in Fall 2007
should submit all application materials no later than February 28, 2007.
1. For admission requirements, please visit the Pharm. D. Requirements pages.
2. Applications must be submitted electronically. Click on 'Apply Electronically'
and follow the instructions. Applications may be saved and accessed at any
time until they are submitted.
3. Please complete the application form prior to submission of transcripts,
letters of recommendation, and pre-professional evaluations.
4. There is a $50 application fee that may be paid via credit card or check
made payable to the UT Health Science Center. Checks and all supporting
documents (transcripts, pre-professional evaluations, and letters of
recommendation) should be mailed to:
